Before filling out a registration form, check you are in our catchment area.
You can also find out which GPs you can register with on the NHS website.
Registering with the surgery
To register with the surgery:
- complete the NHS: register with a GP – Abbey Field Medical Centre
- complete the NHS: register with a GP for Elmstead Market Surgery and Hawthorn Surgery
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Are you on regular medication?
Newly registered patients are required to have a medication review with a GP before medication will be issued. Before registering with us, please contact your current GP surgery and request 4 weeks medication, this will allow time to submit your new registration forms and arrange a medication review. Please allow 7 working days after submitting your registration form before contacting us to make the appointment.
