To register a new patient you will need to live within our practice boundary.

Are you on regular medication?

Newly registered patients are required to have a medication review with a GP before medication will be issued. Before registering with us, please contact your current GP surgery and request 4 weeks medication, this will allow time to submit your new registration forms and arrange a medication review; please allow 7 working days after submitting your registration form before contacting us to make the appointment.
